Accessibility & Patient Support
We want everyone to be able to access our services and communicate with us in a way that meets their needs.
If you have a disability, sensory impairment, communication need or require additional support when accessing our services, please let us know. We will work with you to understand what support or reasonable adjustments may help.
Tell us about your communication needs
If you have particular communication or information needs, please tell a member of our team so that we can record these on your patient record.
For example, you may need:
- information in a different format, such as large print
- support because of a hearing or visual impairment
- information communicated in a particular way
- support from a communication professional or interpreter
Once we know about your needs, we will aim to make sure they are recorded and taken into account when we communicate with you.
Reasonable adjustments
If you have a disability or health condition that makes it more difficult for you to access our services, we can consider reasonable adjustments to help.
These will depend on your individual needs and could include:
- additional communication support
- support when attending the practice
- adjustments to how we communicate with you
- allowing additional time or assistance where appropriate
- considering your individual needs when arranging your care
If you need a reasonable adjustment, please tell us what would help you. We will discuss your needs with you and record appropriate adjustments on your patient record.

You can speak to us in person at either practice, telephone us on 01274 484111 or email us at B83012.manormp@nhs.net or tell a member of our clinical team about your needs during an appointment.
If your needs change, please let us know so that we can keep the information on your patient record up to date.
Interpreters
If English is not your first language and you need an interpreter for an appointment, please let us know when arranging your appointment.
We can arrange a professional interpreter where appropriate.
You do not need to rely on a family member or friend to interpret for you.
For some consultations, particularly where sensitive or clinical information is being discussed, a professional interpreter would be more appropriate.
