If you suffer from one or more long term conditions such as Asthma, Hypertension, COPD, Heart Disease, Diabetes, etc. you will be offered a review with one of the nursing team at the practice.

This appointment is important because we can assess whether your condition has changed and that you are on the correct medication.  Before your appointment try to have a think about what has happened since your last appointment – anything that might help your healthcare professional to know about.  For example, a heart attack, a new job that means a change in mealtimes or shift swaps, etc…

The reviews are usually on an annual basis, you will be advised in the appointment if this is different.  Please contact the practice if you think you're due a review or have any queries regarding your appointment.
